PRE-REGISTRATION IS ADVISED DUE TO SLOTS FOR RIDERS FILLING UP!!

To sign up to compete you must do the following:

1.  Download and print-off the waiver/sign-up sheet below

2.  Collect at least $30.00 or more of donations from a sponsor or a collective donation

**Example-  Get three friends to give you $5.00, get a company to sponsor you, have family members collectively donate, you yourself can pay the entry fee, etc.  This is for a charitable event, so if a company or person needs tax exempt forms or receipt please let us know.  The more donation funds YOU raise, the more the cash prizes are for the winners!  Sixty percent of the proceeds will be donated to the Special Olympics-Southeastern Wisconsin and 40% of the proceeds get divided up for cash prizes!!  The goal is to get as much in donations as possible!!

3.  Email cbairdcoulter@hotmail.com to sign-up.  IT IS RECOMMENDED TO PRE-REGISTER!!  When you email us, put "Polar Jam" as the subject then please give us your name, phone number, address, and age.  The forms and donations can be collected two ways:  mail orders or on the day of the event.  ANY QUESTIONS CALL 262-581-5844.

4.  Show up at 1:00pm on Saturday, February 19th, 2011 to shred and show our judges what you tricks are up your sleeve!!
